Carpal tunnel syndrome
When night-time tingling interrupts sleep and your hand feels less dexterous.
01
What it is
The median nerve passes through a narrow space at the wrist. Compression can affect sensation and hand function. Symptoms and the condition of the nerve guide treatment.
02
Symptoms
Night-time tingling commonly affects the thumb, index, middle and part of the ring finger. Clumsiness and weakness may develop. Little-finger numbness suggests another possible cause, including ulnar nerve compression.
03
Assessment
Assessment combines your history and examination. Ultrasound or nerve conduction testing may help answer specific questions. Not every patient needs the same investigations.
04
Non-surgical treatment
Selected cases may benefit from a neutral-position night splint, activity changes or an injection for symptom relief. Progressive weakness or persistent sensory loss warrants prompt assessment.
05
When surgery is considered
When surgery is indicated, I mainly use endoscopic release. A small camera guides division of the roof of the tunnel. With a single portal, the incision lies near the wrist crease, away from the heel of the palm. Compared with conventional open surgery, this may reduce scar tenderness and improve early recovery. Alternatives and risks, including nerve injury and persistent symptoms, are discussed before surgery.
06
Recovery and follow-up
Surgery is usually a day-case procedure. Tingling, sensation and strength may improve at different rates, depending on pre-existing nerve damage. Dressing care, movement and return to activities are planned individually.
Frequently asked questions
Does the website questionnaire diagnose this condition?
No. It records symptoms and everyday difficulties; scores must be considered alongside clinical assessment.
Is the scar in the palm?
A single-portal approach places the incision near the wrist crease. Palm discomfort can still occur during recovery.
When can I return to work?
Keyboard work and heavy manual work place different demands on the hand. We will plan suitable adjustments and time off according to your job and recovery.
Can I move my hand straight away?
My usual plan uses a dressing and immediate movement, with light tasks as tolerated. Movement does not mean unrestricted loading.
When can I drive?
When you can control the vehicle and respond safely in an emergency, without limiting dressings or impairing medication. This is reviewed individually.
Will I need therapy or stitch removal?
I usually use absorbable sutures. Hand therapy is considered for persistent stiffness, pain or functional difficulty rather than prescribed routinely.
Why are my fingers still numb after surgery?
Release does not immediately reverse previous nerve damage. Sensation may improve more slowly or remain incomplete. New or worsening symptoms need review.
